At Summit Community Care, our main job is to help you keep track of your health and maintain your independence. And we’ll help you every step of the way, with one-on-one support.

 
 

Here’s how it works

Your care coordinator

Your care coordinator is your first point of contact when you have questions or concerns. They’ll get to know you, the people you choose to involve in your care and your doctors.

Your care coordinator will plan to meet with you once a month, or more often if needed, and can help you:
• Learn about your health condition and helpful resources
• Get answers to your questions
• Get the services and supports that work best for you
• Find doctors and other providers who will give you the services on your care plan

Your care team

Your care team is everyone involved in your care — your care coordinator, your doctors, the people you choose to involve in your care, and most importantly, you! We’ll work together to help you get the services and supports you need to live healthy.

You and your care coordinator will work together to figure out what kinds of services you need, who you get them from and how often you need them. You’ll have a care plan designed just for you!

Your health plan support

In addition to your care coordinator, our team of experienced nurses, social workers and behavioral health clinicians knows the community, so they can help you learn about resources available to you. If you need guidance, they can help you understand your options and how you can get the most from your benefits.

  • Your needs change
  • You need help getting to the doctor or managing your medicines
  • You go to the hospital or emergency room or need urgent care

If you’re a new Summit Community Care member, expect a call from your care coordinator to get you started with care management.
